Appointment requests

Submitting the appointment form creates a request, not a scheduled appointment. Our team will contact you to confirm a time. No treatment relationship is created by submitting a form.

Referral submissions

The referral form is intended for use by dental professionals and their staff. By submitting, you confirm you are authorized to share the patient's information for treatment purposes. Uploaded files are stored privately and are accessible only to authorized Transcend team members.

Use of this site

You may not attempt to gain unauthorized access to any part of this site, submit false or automated form data, upload malicious files, or use the site in any way that interferes with its operation.

Content and links

Text, photography, and marks on this site belong to the practice or are used with permission. Links to third-party services — including patient forms, bill pay, and financing partners — lead to sites we do not control and whose terms and privacy practices govern their use.
